Client Services Coordinator
Responsibilities/Duties:
- Determine client eligibility for the program and services that best meets the client needs
- Maintain accurate client records
- Conduct in-person assessments and annual reassessments
- Maintain a wellness call procedure and implement as needed
- Establish a call checklist to identify areas of concern regarding a client’s wellbeing
Qualifications:
- High School diploma minimum education with no less than five years qualifying experience
- Undergraduate Degree in Social Work or Gerontology strongly recommended
- Experience working with the elderly in aging programs preferred
- Excellent verbal and written communication skills a requirement
- Basic knowledge and understanding of aging process a requirement
- Ability to work collaboratively with colleagues
- Organizational skills are critical
- Computer skills and familiarity with Word, Excel, and Client Database software are critical
Time Commitment: During the hours of 8am-2:30pm
Pay is commensurate on experience
